Telephonic Triage with a seamless transition into Nurse Case Management
Telephonic Nurse Case Management
Our nurses are available 24 hours a day, 7 days a week to assess workers' injuries and to make recommendations for next steps to ensure that they receive appropriate care for their injuries. Many times patients are unsure what they should do following an injury, and our nurses are there to provide advice to help them to make decisions that are in their best interest.

  • Our nurses perform a Nurse assessment of patients to ensure that the right treatment is provided at the right time in the right place for the right reasons.
  • Our experience shows that approximately 50% of injuries are resolved with patient administered “home care” advice given by the nurses that can prevent frustrating waiting times in physician offices and emergency rooms when they are unnecessary.  When this is appropriate for the injury a Workers Compensation claim and an OSHA recordable incident are avoided.
  • If physician treatment is appropriate, referrals are made to pre-screened medical facilities. These referrals are done in a manner consistent with pertinent state physician choice regulations.
  • Injury specific details and demographic information are collected for filing of 1st reports of injury if necessary.
At The Health Bridge, we take care to ensure that the patients we serve receive appropriate, effective care for their occupational injuries.  When this is accomplished, there is a demonstrated reduction in the number and cost of Workers Compensation claims and improvement of performance indicators for our client companies.

Our customizable approach allows you to choose the services that are right for your company.
For those injuries that are more serious, our Nurse Case Managers are there to serve the patients to make sure that their needs are being met during their healing process.  Among the many functions that our Case Managers provide are:

  • Acting as a patient advocate
  • Making sure that the right providers are being used
  • Coordinating returns to work and light duty
  • Acting as a hub of communication
  • Assuring a positive process
  • Ensuring compliance with treatment plans
  • Creating expectations for all involved
